The positions shown are the positions
the players are in at the time of the
attack, not necessarily their starting
positions.
The RB is always the setter (assuming
the team is running a 6-2).
The players covering the spike form two
semicircles to cover the attacker.

Attackers NEVER reach back to play a blocked ball. It is the responsibility of the
players covering the spiker to dig those balls.
Covering players should be able to reach down and touch the floor. Otherwise they
are standing too upright and will never be able to dig a ball blocked straight down.
